Cheerleading Forms
These forms can be opened and printed from this page. If you are unable to print them we do have copies in the Recreation building. All forms must be handed in when you pick up your child's uniform.
Uniform Bond
Please fill out the following form and hand it in with a check for $100. This will not be cashed unless you do not hand in your uniform or if there is damage to your uniform. Please do not date the check.
Photo Release
Please fill out the following form and hand it in along with your other documents. This release will allow us to post photos of your child on our website as well as our Facebook pages. Please note that our sites are monitored for members and are not open for the general public to pull.
Concession Bond
Please fill out the following form and hand it in with a check for $100. This will not be cashed unless you do not fulfill your hours for the concession stand. Please do not date the check.
